Remote Patient Monitoring and the Management of Chronic Conditions

| Abstract | The increasing prevalence of chronic conditions has created significant challenges for healthcare systems worldwide, requiring continuous monitoring, timely intervention, and effective long-term disease management approaches. Traditional healthcare models often depend on periodic clinical visits, which may limit continuous observation of patient health conditions. Remote Patient Monitoring (RPM) has emerged as an innovative digital health approach that enables real-time collection, transmission, and analysis of patient health information outside traditional healthcare settings. This study examines the role of Remote Patient Monitoring in the management of chronic conditions. The research explores how remote monitoring technologies, real-time health tracking, early risk identification, patient engagement, and healthcare provider connectivity influence chronic disease management outcomes. A conceptual analytical framework supported by simulated observations is developed to evaluate the contribution of RPM technologies toward improving chronic care delivery. The findings indicate that Remote Patient Monitoring improves chronic disease management by supporting continuous health observation, reducing avoidable healthcare visits, improving medication and lifestyle management, and enabling timely clinical interventions. However, challenges including technology accessibility, data privacy concerns, digital literacy limitations, healthcare integration barriers, and patient engagement issues continue to influence successful implementation. The study contributes to digital healthcare and chronic disease management research by highlighting the importance of remote monitoring technologies in developing patient-centered healthcare models. The findings provide practical implications for healthcare organizations, technology developers, clinicians, and policymakers seeking effective strategies for improving chronic care outcomes. |
